Fillable SSA Disability Forms

I HAVE MOVED

…and joined forces with ssdfacts.com to create a place where SSDI/SSI applicants can find everything necessary to find their way through the maze of the disability application process. As part of that effort, I have expanded the collection of forms to include ALL forms an applicant and/or beneficiary may need through the application process and beyond.

    has been updated as of Jan 2010.

    Can you provide a current FILLABLE version of this form?

    Thank you so much for your site and forms. They correlate exactly with what I will need for SSDI. I have the application in 2-24-10 and hand delivered 96 pages of psych records to the SSI Office.
    Is there any reason I shouldn’t give them my completed
    “Adult Function Report” ?

      Thank you Greg. I’m glad you found them useful.

      I hesitate to give advice, but I would hold the Adult Function Report until you actually receive the request from them to complete it.

      It is something you typically won’t receive until they are at step 3 or 4 of the sequential process. You never know, you might be approved at step 1 or 2.

    I just used the Work History Report it is great. I am having problems finding a fillable SSA-3373-BK (12-2009)ef (04-2010). It contains substantial changes from the 2004 edition. It has an additional question in Section B. 5. How do your illnesses, injuries, or conditions limit your ability to work. Thought you should know about the changes. Thanks

    has been updated as of SSA-3373-BK (12-2009) ef (04-2010) Destroy prior editions it says on the form.

    Can you provide a current FILLABLE version of this form?

    The SSA-3373-BK form that you have posted is no longer being accepted. They said to destroy all old forms.

    You certainly don’t owe me an apology. Thank you so much for your help. The forms work great. If anyone is having any problems with any forms, you should tell them that they should download them first. The fillable forms only worked on my computer once they were downloaded. Then they worked like a charm.

    Kali, I’m not sure what the problem is. I’ve tried it from a couple of different computers and I can load the document OK. I’ve sent you a copy of the Organizing document by email. Please let me know if you don’t get it. Also let me know if you need any of the other documents.

    Your site is truly wonderful. Thank you so much for taking the time to compile the correct and updated forms. Do you suggest that the initial SSDI application be done online or downloaded in hard copy and hand written and turned into the SS office in person? Does it matter either way? Also, did you have an attorney help you with any step of your process and if so, when? I have a list of many other questions —which I can’t believe are not answered even yet….after hours and hours of research on the computer. I would like to contact you via e-mail directly and hope to reach you then. Thanks !

    Your site and what you have done here is truly amazing. Thank you. My questions mirror the above. I downloaded the 5 forms needed to apply, but didn’t notice the application for disability. I have started the online process, and it provides just 2 forms to submit, the application for disability and the adult disability report. If I do decide to scrap my online stuff and bring everything in person, isn’t there an app for disability that needs to be completed?

      Hey Ti:

      I’m glad you’ve found everything helpful. I realize now I should have responded here to answer Kristina’s question. She had also emailed me, so I responded through email.

      To answer the question: The forms are all current. You can see the effective date in the lower left corner. Typically forms will remain active for 3 years after the effective date.

      There was a paper application that is still used on rare occasion, and only when mailed directly from an SSA office. Currently SSA only uses the iClaim (what you are in the process of completing) and the telephone or in person interview. When you complete the iClaim, you will be asked to provide an electronic signature, which is simply clicking a button titled “Sign Now”. Once you have clicked the Sign Now button, you will be given a receipt for your application. In both the telephone and in-person interview, you will be asked to ATTEST to your signature. You won’t physically sign anything at that point either, but your attestation will be entered into the computer.

      I am in the process of moving and expanding my site to include every form an applicant may need for appeal and those beneficiaries may need after approval. It will also include samples of the iClaim, as well as the computer screenshots the interviewer uses when taking an application. They are not complete, but if you would like I can email you the draft I am working from.

    Hi there, thanks for the response. I sent in my iClaim online and got my application number, etc. Now I am in a quandary. Should I send my disability report online as well? Or make an appointment and submit it in person. If I do that I will have to wait until Mid-October for an appointment. I guess I was thinking submitting online would get the ball rolling faster. I am so confused and overwhelmed….(clearly not a Scorpio!) >;<

    You are so clever – I love your signature. How did you ever figure that out?

    If you have already submitted your iClaim, I would go ahead and fill out the Disability Report on-line AND make an appointment.

    When I called to schedule my appointment, it was suggested that before my appointment I complete the online Disability Report. I did that, as well as take the paper version to my appointment.

    The online Disability Report combines the two paper versions of the Disability Report and the Work History Report. So, you might want to complete both paper versions before you start the online version. That way you have all the information at your fingertips.

    I'm not sure if it got the ball rolling any faster or not. My case really got drug out – required CE, sent to QR, etc. All in all, it took 9 months before my approval. And, I was required to complete the Work History Report again before finalizing my claim. I still don't understand that. I just printed my original version and signed it with a different date.

    Each case is so different. I'm hoping your case gets approved quicker than mine did.

    PL, you rock!!! Seriously, the work you have done here is an amazing resource for so many people. (It actually makes me want to cry tears of joy for the good of humanity.) Non-fillable pdf forms are entirely useless and endlessly frustrating. Thank you so much for taking the time to put this site, and especially these fillable forms together.

    One thing, your fillable SSA 827 from is from 2004, but the the current form SSA is using (on their site) was updated in 2009. Just thought you might want to update.
